Welcome to the USCAP Continuing Medical Education website. In order to submit a claim for CME/SAM credit,
you must complete an Overall Course Rating, and also complete an evaluation worksheet for each event you
attended. From the Main Menu, you can click to either the Overall Course Evaluation or the Event Evaluation
Menu. In Mid-March, you will be able to take SAM Tests for any Short courses for which you are registered
AND for which you have completed Evaluation.

This menu lists every session at Practical Pathology Seminars 2008 in chronological order. Simply click on the light blue description of each event you attended to complete the
evaluation for each event. This will take you to the Evaluation Worksheet for each individual event.
At the top of each worksheet it will tell you the requirements for having the worksheet be considered
complete. You need not have attended every speakers' session at an event to claim CME credit, but the CME
credit which will be awarded will depend on the number of individual speaker evaluations we receive. If you
did not attend an individual speaker's session, simply click 'Did not attend'. Once you are finished with a
worksheet, click 'Save Answers' to return to the Event Menu.

Back at the Event Menu, an " " -or- " " flag will appear on this list as soon as you save your answers for each event. This is an
indicator for you to quickly see your progress in completing evaluations.

Click the Main Menu button on the toolbar to return to the main menu where all currently available options are presented. NOTE: The SAM
Test and the CME/SAM Claim Form option will not appear on your main menu until you have completed (i.e. " ") ALL evaluations.

You may come back as often as you like to complete these evaluations and all your responses will be saved, as
long as you click the 'Save Answers' button after entering your ratings/comments for each session.

The SAM Test option will appear on the main menu once you have completed all evaluations.
This year, you may convert CME credit to SAM credit if:
- You have completed the evaluation for each session as well as the Overall Meeting Evalutaion

- You answer at least 80% of the SAM Test questions correctly

You may come back as often as you like to complete the test and all your responses will be saved.
While taking these tests, an " ", " " -or- " " flag will appear on this list to quickly indicate your progress.
Likewise, for tests in process, the % complete will appear next to the icon on the menu below. Once you have completed all the questions, "100% Complete" will appear on the menu below, indicating that you are ready to submit that test.

When you are finished answering all the questions, you must click 'Submit Test' in the toolbar at the top of the test, to have the test scored and to see your results.
At that point the "Passed" or "Not passed" flag will appear in the menu below for that test, and the % correct will be displayed on the menu.
If " ", you may come back to retake this test
by clicking on the short course title as often as you like, but you must score at least 80% correct to pass and have this test applicable for SAM credit.

Once you have completed all your evaluations (and optionally the SAM test) - you need to go to the CME/SAM Claim Form to claim CME credit and convert any CME credit to SAM for which you are eligible.

This Claim Form has been built for you based on the Evaluations you made for each session you attended.

In general, if this CME/SAM Claim Form does not accurately reflect the events you attended,
please return to the Evaluations and make certain you have accurately completed all evaluations for events you attended.
You may also click on any of the blue day names below in the form to proceed quickly to that portion of the Evaluation Menu.
If, when you return to the CME/SAM Claim Form, inaccuracies
still exist, please email the help desk
for help on this.

At the bottom of the CME/SAM Claim form, if you have taken and passed the SAM Test, you will be able to convert any number of CME hours (up to the maximum accredited)
to SAM credit. Whatever value you enter here will be subtracted from your CME Credit Total and added to your SAM credit Total.

Once you are satisfied that this form accurately reflects the events you attended,
enter your Electronic Signature at the bottom of the page and then
click the 'Print CME/SAM Claim Form' below to print for your records. Then you must click 'Submit to USCAP' to have your "CME Hours Claimed" and "SAM Hours Claimed" transmitted to us for your transcript.
These hours will be available to you in your Member Portfolio after August 1, 2008.
